Job summary

Merakey is seeking a Blended Case Manager to join our team in Monaca, PA. You will serve as the primary contact for up to 30 community-based individuals, developing personalized service plans and coordinating care with multiple providers to support each person’s goals.

The role includes counseling, on-call duties, and collaboration with county reentry initiatives to assist clients transitioning back into the community.

Qualifications

• High school diploma or GED and at least 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science required.
• 2+ years’ experience in public or private human service, with 1 year direct care required, with clients over 18.
• Valid Driver's license required.
• All clearances required by OMHSAS regulations.
• Bachelor's degree in sociology, social welfare, psychology, gerontology, or other human services preferred.

• Offer supportive counseling to help individuals achieve their goals.
