Job summary

Merakey seeks a Blended Case Manager (BCM) for our Philadelphia, PA program. Earn $23.50 per hour as a BCM, with $24.60 per hour for a Bilingual BCM.

You will develop, coordinate, and implement services according to each consumer's service plan, addressing health, family functioning, education, and social goals. The ideal candidate holds a relevant Bachelor's degree, has a valid driver's license for travel between work locations, and may benefit from bilingual English/Spanish proficiency.

Qualifications

• Bachelor's degree in psychology, social work, nursing, rehabilitation, counseling, or related field is required.
• Valid driver's license and reliable transportation with access to a vehicle during working hours are required for travel between locations.
• Experience in human services, especially with children, is preferred.
• Bilingual proficiency in English and Spanish is preferred.

Responsibilities

• Develop, coordinate, and implement services for assigned consumers as identified in their service plan.
• Assist consumers across life domains: health, family functioning, education/vocational goals, and socialization.
• Work with teams to ensure effective service delivery and consumer outcomes.

Position Overview:

The Blended Case Manager (BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The BCM will work and assist their assigned consumers by focusing on the following life domains:

• Behavioral and physical healthcare needs
• Family functioning/independent living
• Educational and vocational goals
• Socialization
