Job summary

Merakey in West Lawn, PA is seeking a full-time Case Manager to join our Dual Diagnosis Treatment Team. You will assess, plan, and support recovery for adults and youths with mental health diagnoses or autism, with a focus on a recovery-oriented, person-centered approach.

The role involves initial intakes, collaborating with the team, and providing individual and group treatment across diverse settings. Travel within the community is required, and Merakey offers robust benefits and growth

Qualifications

• Bachelor's degree in Social Work, Psychology, or another human service-related field is required.
• Must complete clearances and background checks at Merakey's expense.
• Valid PA driver's license with a clean driving record required.
• 1 year of experience in the behavioral health field preferred.
• Candidate must be open to community based travel.

Responsibilities

• Collaborating with other team members on the implementation of a recovery-oriented person-centered treatment and support plan.

• Providing intensive therapeutic services in settings including community living arrangements, schools, and community settings.

Position Details

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum.

The Recovery Coordinator/Case Manager will be responsible for:

• Collaborating with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan.
• Performing initial intakes, assessments, and timeline summaries.
• Providing intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.
• Poviding individual and group treatment and support according to each individual's/family's needs.
• Supporting data collection, monitoring interventions, and providing feedback to the treatment team regarding client progress.
