Job summary

Merakey in Wilkes-Barre, PA is seeking a full-time Case Manager to join the Dual Diagnosis Treatment Team. You will assess, plan, and support recovery for adults and youth with mental health diagnoses, including autism, across community and school settings.

This role pays $24.99 per hour. Merakey offers mileage reimbursement and a company car for travel, plus growth opportunities and a strong benefits package to support well-being and work-life balance.

Qualifications

• Bachelor's degree in Social Work, Psychology, or another human service-related field.
• Valid PA driver's license with a clean driving record.
• 1 year of experience in the behavioral health field preferred.
• Will be required to complete clearances and background checks, at Merakey's expense.

Responsibilities

• Collaborating with team members to implement recovery-oriented, person-centered treatment and support plans.
• Performing initial intakes, assessments, and timeline summaries.
• Providing intensive therapeutic services in settings including community living arrangements, schools, community settings, and natural support residences.
• Providing individual and group treatment and support according to each individual's/family's needs.
• Supporting data collection, monitoring interventions, and providing feedback to the treatment team regarding client progress.

Position Details

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum.

The Recovery Coordinator/Case Manager will be responsible for:

• collaborating with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan.
• performing initial intakes, assessments, and timeline summaries.
• providing intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences.
• providing individual and group treatment and support according to each individual's/family's needs.
• supporting data collection, monitoring interventions, and providing feedback to the treatment team regarding client progress.
