Hackensack Meridian Health is seeking a Transition Team Advanced Practice Nurse (APN) to lead patient-centered, coordinated care across the transition of care process. Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Responsibilities

• Performs comprehensive histories and physical assessments.
• Orders, performs, and interprets labs and studies in collaboration with physicians.
• Manages general medical/surgical conditions using evidence-based practices.
• Prescribes medications safely per NJ regulations and provides patient education.
• Promotes APN practice recognition through conferences, publications, and presentations.
• Provides patient- and family-centered care adhering to nursing protocols and standards.
• Participates in daily interdisciplinary rounds to ensure coordinated care.
• Mentors APNs and precepts students in APN programs.
