Job summary

Hackensack Meridian Health Inc. is seeking an Advanced Practice Nurse to join the Transition of Care Unit as a part-time Nurse Practitioner. The position offers 20 hours per week, including every Saturday and Monday, with benefits.

The candidate will hold an NLN/AACN accredited nursing degree and an NP credential, with prescriptive authority and a collaborative physician agreement. Strong communication and computer skills are essential for coordinating patient care across teams.

Qualifications

Graduate of an NLN/AACN accredited nursing program.
Master's or Doctorate degree from an accredited Nurse Practitioner program.
Maintains clinical competencies through continuing education and clinical practice.
Experience in a relevant patient care setting (or as regulations dictate).
National Provider Identifier (NPI).
Prescriptive authority through CDS and DEA registration according to New Jersey state law.
Collaborative agreement with physicians on staff.
Excellent written and verbal communication skills.
Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Responsibilities

The APN utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.
