Job summary

Hackensack Meridian Health is seeking an Advanced Practice Nurse to join our outpatient primary care team in New Jersey. The APN will lead patient-centered care, collaborate across disciplines, and advance evidence-based practice.

Responsibilities include comprehensive assessments, diagnosing, prescribing within NJ regulations, care coordination, and mentoring staff. Board certification and NJ NP license required; strong communication and leadership essential.

Qualifications

• Graduate of NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited NP program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills that include Microsoft Office and/or Google Suite.

Responsibilities

• Performs and documents comprehensive history and physical assessments.
• Orders, performs, and interprets labs and diagnostics with physicians.
• Manages general medical and surgical conditions with evidence-based care.
• Prescribes medications safely per NJ regulations and patient education.
• Promotes APN practice recognition through conferences and publications.
• Provides patient- and family-centered care.
• Adheres to nursing protocols and quality indicators.
• Applies evidence-based and value-based medicine to improve care outcomes.
• Assesses and plans transitions of care considering cost, safety, and resources.
• Mentors APNs and precepts students.
