Job summary

CT Community Care helps people live their best lives at home with active community connections. This role provides information and outreach for the MFP program, facilitates transitions, and handles all related data entry and reporting.

It establishes relationships with consumers, families, facility staff, DSS social workers, and community providers, conducts initial interviews, and supports transition plans with 365-day follow-up and record maintenance.

Qualifications

• Bachelor’s Degree preferred in Human Services or related field.
• High School Diploma or equivalent required or preferred with related experience.
• Thorough knowledge of community resources.
• Ability to work as part of a team and communicate effectively.

Responsibilities

• Provides information and outreach for the MFP program and handles related data entry and reporting.
• Establishes relationships with consumers, families, facility staff, DSS workers, and community providers.
• Acts as liaison between providers, DSS, and the State transition program.
• Conducts initial interview and completes intake for skilled nursing facility residents.
• Assists in development and implementation of transition plans and follows up for 365 days.
