Connecticut Community Care is seeking a dedicated outreach coordinator to support consumers in the MFP program. The role emphasizes relationship building with families, facilities, and DSS, plus administering intake and transition plans with accurate data entry and reporting.
The position requires a High School Diploma or better, with a Bachelor’s preferred, and reliable transportation to cover multiple sites. Strong teamwork and MS Office proficiency are essential.

Provides information and outreach support for consumers on the MFP program. Facilitates the transition of eligible individuals and performs all related data entry and reporting.
