Job summary

Kern Regional Center is seeking a dedicated Service Coordinator/Case Manager to coordinate services for individuals with developmental disabilities. You will develop and implement IPPs, PCPs, and annual reviews while advocating for clients and their families.

The role requires a BA/BS in a human services field and at least two years of relevant experience, with Spanish/English bilingual skills highly desired.

Qualifications

• BA/BS degree in human services or related field and two years of relevant experience.
• Bilingual Spanish/English preferred.
• Detail-oriented with strong organization and computer skills.
• Strong written and verbal communication; ability to provide responsive customer service.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide overall service coordination, advocacy, and services arrangement for consumers and families.
• Develop and implement IPP and PCP, plus progress reports and annual reviews.
• Perform follow-up, reassessments and periodic progress reviews.
• Identify and secure community resources and referrals.
• Initiate vendored services and monitor service delivery, while staying updated on regulations.
