Job summary

Qualifications

• Bachelor’s or Master’s degree in Social Work or a related field.
• Excellent written, verbal, and interpersonal communications skills.
• Strong time management and organization skills.
• One year of professional experience in social work or related field (preferred).

Responsibilities

• Provide case management and advocacy for persons with developmental disabilities.
• Assist clients and families in acquiring and maintaining assessed supports and services.
• Develop and monitor Individual Program Plans and coordinate related services.
• Coordinate transportation, vocational training, day programming, education, and health services.
• Advocate for clients with community agencies and vendors; assist with certifications, applications, and referrals.
