Job summary

Alta California Regional Center in Sacramento seeks a Service Coordinator to provide case management and advocacy for individuals with developmental disabilities. You will help clients obtain supports across transportation, training, education, health, and independent living services; serve as an advocate with community agencies; and coordinate with vendors on certifications and referrals.

• Excellent written, verbal, and interpersonal communications skills.
• Strong time management and organization skills.

Responsibilities

• Assist clients and families in obtaining supports and services (transportation, training, education, health, living skills).
• Advocate with community agencies and coordinate vendor certifications and referrals.
• Participate in developing and monitoring Individual Program Plans.
• Complete required documentation and provide on-call emergency coverage.
