McKinley ECC & SNAP in South Bend, Indiana is seeking a SNAP Social Worker to support students and families within the SBCS community. The role reports to the SNAP Building Principal/SNAP Supervisor and requires coordination of transition meetings, evaluation scheduling, and data management for two full-time teams.

The ideal candidate holds a Master’s in Social Work (or related fields), with experience in early childhood special education preferred, and a proven ability to communicate clearly

Qualifications

  • Master’s in Social Work/Master’s in Counseling and/or Master’s in Special Education required.
  • Knowledge of disabilities and resources for early childhood special education preferred.
  • Experience working with diverse populations and with families, students, and community members.

Responsibilities

  • Coordinate and attend transition meetings as the district’s LEA representative.
  • Coordinate schedules, data and timelines for evaluation teams.
  • Complete intakes for full evaluation referrals.
  • Coordinate medical information and health plan implementation with school nurses and teachers.
  • Manage referrals and serve as liaison between school and families.
  • Attend case conferences and tours for new families.
