Job Description

Position Title: SNAP Social Worker

Immediate Supervisor: SNAP Building Principal/SNAP Supervisor

Qualifications:

  • Master’s in Social Work/Master’s in Counseling and/or Master’s in Special Education
  • Special Education experience or licensure is preferred
  • Knowledge of disabilities and available resources for early childhood special education
  • Ability to work well with students and parents (Proven record of ability to work well with children, families, community members, and colleagues in a high-paced environment)
  • Proven record of ability to communicate clearly in a friendly and professional manner in person, by phone, and in emails with colleagues, families, and community members from other organizations
  • Experience working with diverse populations
  • Proven record of ability to work well under pressure and meet timelines consistently
  • Proven record of accuracy and attention to detail in collecting and communicating data
  • Professionalism in appearance and demeanor

Duties:

  • Coordinate and attend First Steps Transition Meetings as the school district’s LEA representative
  • LEA representative for cluster B First Steps Transition Committee Meeting
  • Coordinate schedules, paperwork, data and timelines for two full-time multidisciplinary evaluation teams
  • Complete intakes for full evaluation referrals after being screened by the SNAP Administrative Assistant
  • Coordinate medical information for new students/referrals; oversee that health plans are written and implemented in collaboration with school nurses and teachers
  • Work closely with the Preschool Coordinator and Administrative Assistant to ensure that data is managed on all students from referral to enrollment; ensuring that timelines are met
  • Manage the SNAP Program email account
  • Coordinate social services to students, parents, administrators, teachers, and community agencies
  • Head Start liaison for referrals and services
  • Manage McKinney-Vento referrals as needed
  • Manage and provide support to students and families on the IPV list
  • Serve as point of contact for doctor referrals, community preschool referrals, and daycare referrals
  • Serves as a member of the teacher support team; regularly attends staffings with teachers to provide support for FBAs and BIPs
  • Submit abuse and neglect referrals to the Department of Child Services
  • Referral source for Christ Child and other community resources
  • Attend case conferences as needed
  • Schedule and conduct McKinley Early Childhood Center tours for all families new to the district or transitioning from First Steps
  • Responsible for maintaining resources and materials in the SNAP Family Room
  • Support teachers and students during arrival and dismissal
  • Support teachers throughout the day as needed
  • Liaison between school and home
  • Other duties as assigned by the SNAP building principal
