Job summary

Rockport Healthcare Services is seeking a licensed Occupational Therapist to provide direct patient care and support the therapy department’s outcomes. The role involves evaluating patients, developing treatment plans, and supervising assistants in geriatric rehabilitation settings.

You will document care according to regulatory and billing requirements, communicate with families and medical staff, and contribute to team conferences and professional development.

Qualifications

• Current OT licensure in state required.
• Experience in geriatric rehabilitation preferred.
• CPR certification.
• Excellent verbal and written communication skills.

Responsibilities

• Perform accurate, comprehensive patient evaluations.
• Develop treatment plans and obtain physician approvals.
• Supervise OT assistants and aides per policy and regulations.
• Communicate patient progress with supervisor and health team members.
• Educate families or nursing staff on follow-through programs.
• Interview patients and families about prior functioning and goals.
• Document for regulatory, payor, and accrediting requirements with objective outcomes.
• Contribute to interdisciplinary patient care and team conferences.
• Educate team members and assist with discharge planning.
