Job summary

HCA HealthOne seeks an experienced RN Transfer Center Coordinator in Denver for 7pm-7am shifts. You will manage patient placement, coordinate transfers across facilities, and ensure efficient bed management while maintaining high-quality patient care.

You will collaborate with physicians, nurses, and administrative staff, using strong communication and clinical judgment to optimize throughput and patient safety in a fast-paced hospital network.

Qualifications

• Graduate of an accredited nursing program required; BSN preferred.
• Minimum of 3 years’ related clinical experience in an acute care setting (ED or Critical Care preferred).
• RN License in the states where the HCA facilities for that Division are located – multi-state license required if Transfer Center covers more than a single state (may be obtained upon hire).
• Knowledge of the principles and practices of hospital and clinical operations and patient care practices.
• Demonstrated skills in problem solving, analytical and critical thinking, prioritization, negotiation, conflict resolution, proactive decision making.
• Ability to establish and maintain collaborative and effective working relationships.
• Proficient in Microsoft Office applications, Internet searches, and relevant databases.

Responsibilities

• Performs all transfer center functions with centralized patient placement.
• Coordinates transfer of emergent, urgent and elective patients to a final destination facility from all referral sources.
• Facilitates patient throughput with focus on quality and efficiency; provides follow-up to participants.
• Works well with others and promotes teamwork.
• Patient placement functions ensure timely bed placement and accurate initial care plan.
• Maintains data collection and workflow in patient records and systems.
• Communicates with referring facilities, physicians, and receiving facilities about transfers.
