Job summary

Medical City Orthopedic and Spine Surgery Center in Dallas, TX seeks a Total Joint Program Nurse Navigator to coordinate the center's Total Joint Replacement Program from preop through follow-up. You will guide patients and families, assess home readiness, and advocate across departments to ensure seamless care.

You will lead education sessions, monitor outcomes data, and partner with nursing leadership to improve CHAPS scores and overall patient experience within a busy, multidisciplinary

Qualifications

• 5 years applicable clinical experience.
• Minimum 2 years in an acute care orthopedic setting, preferred.
• Supervisory experience preferred.
• Operational and strategic development experience preferred.
• Associate Degree in Nursing or Nursing Diploma required.
• BSN required. (internal candidates only – experience may substitute for degree)

Responsibilities

• Coordinate and implement the center's Total Joint Replacement Program.
• Guide patients and families through preoperative assessment, education, discharge and follow-up.
• Assess patient support systems and home-care readiness.
• Liaise between departments and advocate for the patient.
• Lead pre-operative assessments with the ASC Medical Director and provide education.
• Monitor and develop quality metrics and outcomes data for governance.
