Job summary

Heritage Valley Health System seeks an Orthopedic Clinical Navigator to guide patients through pre-operative evaluation to post-acute care. You will establish communication with patients and families, clarifying roles and expectations for surgery and recovery.

In collaboration with physicians and other care teams, you will identify patient needs, coordinate services, and enhance education to improve outcomes and satisfaction throughout the episode of care.

Qualifications

• Must be a graduate of a professional school of nursing with current Pennsylvania licensure.
• Two years of orthopedic/rehabilitation experience.
• Professional and effective communication and customer relations.
• Excellent public speaking ability.
• Able to adapt and facilitate change.
• Works well with physicians and ancillary departments.
• Proficient with computers and software applications.
• Must complete required clearances within 90 days of employment.

• Coordinate care from pre-operative evaluation through post-acute care.
