Job summary

Qualifications

• Current, active, and unrestricted Registered Nurse (RN) licensure in state of practice.
• Current CPR certification.
• Minimum of one year of RN experience.
• Valid driver’s license and reliable transportation.
• Liability insurance to complete home visits.
• Bachelor of Science in Nursing (BSN) preferred.

Responsibilities

• Provides competent nursing care and interventions to home health patients according to state and industry standards.
• Supervises care by Home Health Aides and LPNs, completing supervisory visits as required.
• Collaborates with interdisciplinary team and physician to develop and manage the plan of care.
• Performs field visits to evaluate start of care and monitor outcomes and performance initiatives.
• Documents progress at the point of care using required systems and timelines.
• Adheres to safety regulations and maintains licensure, certification, and records.
