Job summary

Devoted Health seeks an experienced Advanced Practice Provider (APP) to lead the Medical Transitions of Care program. The APP will conduct comprehensive post-discharge visits, manage medications, and coordinate care across a highly interdisciplinary team using telehealth.

The role emphasizes building an integrated, whole-person care plan for geriatric patients, contributing to quality-improvement projects, and shaping the EHR as the program scales.

Qualifications

• 5+ years of direct patient care with geriatric populations and multiple chronic conditions.
• Licensure as RN, APRN, or PA with active good standing.
• Experience in internal medicine, urgent care, hospital medicine, or geriatrics is preferred.

Responsibilities

• Conduct comprehensive post-discharge assessments, med rec, and care planning.
• Perform acute/virtual urgent-care TOC visits and determine level of care.
• Build and adjust whole-person 30-day chronic-care plans.
• Participate in interdisciplinary TOC rounds and collaborate with care team.
• Educate team members and contribute to EHR improvements and quality projects.
• Support program development, culture, and communication within TOC.
