Job summary

Devoted Health is seeking an experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to lead the Medical Transitions of Care program. The APP will perform comprehensive post-discharge visits, manage medications, and build a whole-person care plan within 30 days of discharge.

The role emphasizes telehealth visits, collaboration with care coordinators and physicians, and quality-improvement contributions.

Qualifications

• 5+ years of direct patient care experience in geriatric populations.
• Experience in internal medicine, urgent care, hospital medicine, post-acute medicine, geriatrics.
• Telehealth experience using video platforms.
• Strong teamwork in interdisciplinary settings.

Responsibilities

• Conduct post-discharge assessments of medical conditions, meds, functional status, and psychosocial needs.
• Perform post-discharge medication reconciliation and optimize therapy.
• Place orders (prescriptions, labs, imaging, referrals, home health, DME) and provide education.
• Conduct acute/virtual urgent-care TOC visits and determine level of care.
• Adjust plans of care based on evolving clinical picture and barriers to care.
• Participate in TOC rounds and collaborate with RN case managers, care coordinators, social workers, pharmacy, and PCPs.
• Contribute to quality-improvement projects and provide EHR feedback.
• Educate broader team on TOC care and participate in pilots.
