Job summary

Devoted Health is seeking an Advanced Practice Provider (APN/PA) for the Medical Transitions of Care program. You will conduct comprehensive post‑discharge visits, manage acuity through telehealth, and develop whole‑person care plans within a 30‑day window.

Qualifications

5+ years of direct patient care managing geriatric populations with multiple chronic, comorbid conditions across acute and chronic presentations.
Direct care experience in post‑discharge or transitional care.
Experience delivering visits via telehealth.
Ability to work full‑time, Monday–Friday.
Active RN/APRN/PA licensure with ability to obtain required state licenses within 90 days.

Responsibilities

Perform comprehensive initial TOC visits and post‑discharge assessments, reconcile medications, and communicate contingency plans.
Conduct acute/virtual urgent‑care TOC visits and determine level of care.
Manage care trajectory within the 30‑day window, adjusting plans as clinical picture evolves.
Participate in interdisciplinary TOC rounds with care teams and PCPs.
Contribute to quality‑improvement projects and provide EHR feedback.
Adapt and contribute to program growth in a fast‑paced, evolving environment.
