Job summary

Devoted Health is seeking an experienced Advanced Practice Provider (APRN/PA) to lead our Telemedicine Transitions of Care program. The role combines initial comprehensive post‑discharge visits with follow‑up and urgent visits, across a 30‑day window, to prevent readmissions and coordinate complex care for older adults.

You’ll shape the TOC program, collaborate with an interdisciplinary team, and contribute to quality improvement and EHR evolution in a fast‑paced, supportive environment.

Qualifications

• 5+ years direct patient care with geriatrics and multi‑morbid conditions.
• Active RN/APR N or PA license with ability to obtain licensure in required states within 90 days.
• Full-time availability Mon-Fri 8:30 AM-5:00 PM CST/MST/PST preferred.

Responsibilities

• Perform comprehensive initial TOC visits including post‑discharge assessment, med reconciliation, education and contingency planning.
• Conduct acute/virtual urgent‑care TOC visits and determine level of care and necessary interventions.
• Manage clinical trajectory across the 30‑day window and adjust care plan accordingly.
• Participate in interdisciplinary TOC rounds with care coordinators, social workers, pharmacy and PCPs.
• Enhance team culture with clear communication and escalation protocols.
• Educate the team, engage in quality improvement, and provide EHR feedback.
• Be adaptable to expand to new clinical/organizational initiatives in a build environment.
