Job summary

Western Home Communities and Thrivera atHome are seeking a compassionate Home Health Aide (CNA) to join our in-home care team serving clients in Black Hawk, Grundy, Bremer, Butler, and Buchanan counties. You will provide in-home care that promotes independence and quality of life, traveling to clients’ homes in your own vehicle.

You’ll work independently, offering personal care, mobility support, meal prep, light housekeeping, and companionship while coordinating with our office for changes in

Qualifications

• Must have a valid driver’s license and a reliable vehicle with active insurance.
• Must be certified by the State of Iowa as a Certified Nurse Aide (CNA).
• Willingness to perform hands-on care (bathing, mobility, daily activities).
• CPR certification or willingness to obtain it.
• Self-starter capable of working independently and managing a schedule.

Responsibilities

• Provide compassionate in-home care to clients, assisting with daily activities and maintaining independence.
• Assist with personal care and mobility, including bathing, dressing, and transfers.
• Provide light housekeeping, meal preparation, and errands to support home safety and comfort.
• Serve as a liaison with office staff, reporting changes in a client’s condition.
• Use own reliable vehicle to travel to clients’ homes across designated counties.

Requirements of a Home Health Aide:

• (Required) Must have a valid Driver’s License

• (Required) You must have a reliable vehicle and active vehicle insurance, as you will be responsible for travel to the client’s home

• (Required) Must be certified by the State of Iowa as a Certified Nurse Aide (CNA)

• (Required) Willingness to Perform Hands-On Care: You should feel comfortable assisting with personal care and mobility.

• CPR Certification or the willingness to obtain it.

• Self-Starter : You will be working independently, so the ability to move efficiently between tasks and manage your schedule is a must.

What You’ll Do as a Home Health Aide:

As a Home Health Aide, you will be a true hero to those you serve by providing essential in-home care. You will work independently, moving between clients’ homes, and directly impacting their daily lives.

• Provide Compassionate In-Home Care : Assist clients with daily activities, including personal care, meal preparation, light housekeeping, and other tasks to help maintain their independence at home.

• Physical Support for Clients : Help clients with personal hygiene tasks such as bathing, dressing, and mobility (getting in/out of bed, walking, standing, etc.).

• Be a Trusted Support System : Respond to clients’ needs with patience, care, and professionalism. Offer emotional support and reassurance to help clients feel safe and valued.

• Maintain a Clean and Safe Environment : Assist with light housekeeping, laundry, meal prep, grocery shopping, and errands, all while maintaining a safe and organized living space for your clients.

• Use your own reliable vehicle to get to clients’ homes in Black Hawk, Grundy, Bremer, Butler, and Buchanan counties—and help them get where they need to go, like doctor’s appointments, grocery runs, and other errands that help them stay independent.

• Act as a Liaison : Report any concerns or changes in a client’s condition to the office staff, ensuring a smooth flow of communication and the highest level of care.
