Western Home Communities is seeking Home Health Caregivers to travel to clients’ homes in Black Hawk, Grundy, Bremer, Butler, and Buchanan counties. You will provide essential in-home care and support to help clients maintain independence and quality of life.

You will work independently, visit multiple clients, and be a compassionate, reliable presence in their daily routines. CPR or willingness to obtain it is encouraged. A valid driver's license and reliable vehicle are required.

Competenze

• Must have a valid Driver's License.
• Reliable vehicle with active insurance since you travel to clients' homes.
• Willingness to perform hands-on care including personal care and mobility.
• CNA not required but encouraged.
• CPR Certification or willingness to obtain it.
• Self-starter capable of independent scheduling.

Requirements of a Caregiver:

• (Required)Must have a valid Driver’s License
• (Required)You must have a reliable vehicle and active vehicle insurance, as you will be responsible for travel to the client’s home
• (Required) Willingness to Perform Hands-On Care: You should feel comfortable assisting with personal care and mobility.
• Do not need to be a CNA for this role, although it is encouraged
• CPR Certification or the willingness to obtain it.
• Self-Starter : You will be working independently, so the ability to move efficiently between tasks and manage your schedule is a must.

What You’ll Do as a Caregiver:

As a Home Health Caregiver, you will be a true hero to those you serve by providing essential in-home care. You will work independently, moving between clients’ homes, and directly impacting their daily lives.

• Provide Compassionate In-Home Care : Assist clients with daily activities, including personal care, bathing, meal assistance, light housekeeping, transportation and errands, medication reminders/administration and other tasks to help maintain their independence at home.
• Physical Support for Clients : Help clients with personal hygiene tasks such as bathing, dressing, and mobility (getting in/out of bed, walking, standing, etc.).
• Be a Trusted Support System : Respond to clients’ needs with patience, care, and professionalism. Offer emotional support and reassurance to help clients feel safe and valued.
• Maintain a Clean and Safe Environment : Assist with light housekeeping, laundry, meal prep, grocery shopping, and errands, all while maintaining a safe and organized living space for your clients.
• Use your own reliable vehicle to get to clients’ homes in Black Hawk, Grundy, Bremer, Butler, and Buchanan counties—and help them get where they need to go, like doctor’s appointments, grocery runs, and other errands that help them stay independent.
• Act as a Liaison : Report any concerns or changes in a client’s condition to the office staff, ensuring a smooth flow of communication and the highest level of care
