Job summary

PAX Health, LLC is seeking a Lead Patient Care Coordinator – Therapy Services to support daily therapy operations, coordinate therapist workflows, and act as the central liaison between leadership and therapists. The role ensures smooth scheduling, clear communication, and an exceptional patient experience throughout treatment.

Key responsibilities include onboarding/offboarding therapists, managing calendars, handling referrals and documentation workflows, and HIPAA-compliant reporting.

Qualifications

• High school diploma or equivalent.
• Healthcare Administration certificate program preferred.
• 3+ years of experience in behavioral health, patient coordination, or healthcare operations.
• Proficiency in Word, Excel, and Google Suite.
• Proficiency with scheduling systems and EHR platforms (Charm, PAX).
• Ability to manage multiple priorities in a fast-paced environment with empathy.

Responsibilities

• Serve as the primary liaison with leadership and Therapist Leads to align priorities, workflows, and needs.
• Facilitate communication between therapy providers, front-desk staff, and leadership for unified patient care standards.
• Oversee onboarding/offboarding of therapists and scheduling setup; coordinate transitions.
• Manage therapist scheduling and calendar accuracy to ensure patient flow.
• Respond promptly to therapy-related communications and follow up on inquiries.
• Conduct outreach for missed appointments and new-patient confirmations; support retention.
