Job summary

Health Connect America seeks a licensed mental health Therapist to provide high-quality services to clients in homes, communities, and offices. You will perform assessments, develop treatment plans, document progress, and collaborate with a multidisciplinary team.

This role emphasizes outreach to attract referrals, crisis intervention, and adherence to standards. Requirements include Master’s in Social Work, Counseling, or related field and licensure eligibility (state varies).

Qualifications

• Master’s degree in Social Work, Counseling, or related field is required.
• Must be eligible for licensure as LPC, LMFT, or LCSW in at least one state.

Responsibilities

• Perform biopsychosocial assessments to determine needs and interventions.
• Create individualized treatment plans with individuals, families, and the treatment team.
• Deliver therapy in individual, group, and family formats.
• Document all interactions, treatment plans, and progress per policy.
• Coordinate with other professionals as part of a multidisciplinary team.
• Engage in outreach to promote services and attract referrals.
• Provide crisis intervention and develop safety plans as needed.
