Job summary

Health Connect America is seeking a licensed or licensure-eligible therapist to provide high-quality mental health services to clients across homes, communities, and offices. You will conduct assessments, develop personalized treatment plans, and collaborate with a multidisciplinary team to deliver evidence-based care.

Qualifications

• Master’s Degree in Social Work, Counseling, or related field required.
• Must be eligible for LPC, LMFT, or LCSW licensure (state-specific).
• Licensure eligibility varies by state (AL, MS, NC, SC, TN, VA).

Responsibilities

• Perform biopsychosocial assessments to determine needs and interventions.
• Create individualized treatment plans with individuals, families, and the treatment team.
• Deliver individual, group, and family therapy sessions.
• Document all client interactions, treatment plans, and progress notes within required timeframes.
• Ensure regulatory and organizational compliance.
• Engage in outreach to promote services and attract referrals.
• Participate in regular treatment team meetings to discuss progress and strategies.
• Provide crisis intervention and safety planning; coordinate with emergency services as needed.
• Stay current with best practices and pursue ongoing training.
