Job summary

Health Connect America seeks a licensed or licensure-eligible Therapist to provide high-quality mental health services. You will assess, plan, and implement evidence-informed treatment for clients across our care settings, including home, community, and office-based services.

You will collaborate with a multidisciplinary team to deliver individualized care, document progress, and participate in outreach to promote programs.

Qualifications

• Master’s Degree in Social Work, Counseling, or a related field is required.
• Must hold/obtain licensure as LPC, LMFT, or LCSW where applicable.

Responsibilities

• Perform biopsychosocial assessments to determine needs and appropriate interventions.
• Create individualized treatment plans with individuals, families, and the treatment team.
• Deliver therapy in individual, group, and family sessions.
• Accurately document client interactions, treatment plans, and progress notes per policy.
