Job summary

Health Connect America is seeking a licensed Therapist to provide comprehensive mental health services to children and families within a hybrid model. The role emphasizes assessment-driven, evidence-based care and collaboration with multidisciplinary teams to promote measurable progress.

Applicants should hold a Master’s Degree in Social Work, Counseling, or a related field, be eligible for Georgia licensure (LPC/LMFT/LCSW), and have at least two years of relevant experience.

Qualifications

• Master’s Degree in Social Work, Counseling, or a related field is required.
• Minimum of two years’ experience working with children and/or families, which may include internships.
• Must be eligible for professional licensure in the state of Georgia as a LPC, LMFT, or LCSW.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and develop individualized, goal-oriented treatment plans
• Deliver evidence-based therapy services through individual, group, and family sessions to address a range of mental health needs
• Maintain consistent engagement with clients, monitoring progress and adjusting treatment approaches as clinically indicated
• Provide crisis intervention, including risk assessment, safety planning, and coordination with emergency supports when needed
• Collaborate with multidisciplinary teams and connect clients to community resources to ensure holistic, coordinated care
• Complete accurate, timely documentation and maintain compliance with all regulatory, ethical, and organizational standards
