Job summary

Habitat Health is seeking a Recreational Therapist (CTRS) to support participants in Habitat Health’s PACE program. You will plan and deliver therapeutic recreation interventions, work as part of an interdisciplinary team, and contribute to individualized care plans to promote functional ability and quality of life for older adults.

The role involves documenting assessments, adapting activities for diverse needs, and ensuring safe programming across day center and home settings.

Qualifications

• Must be a Certified Therapeutic Recreation Specialist (CTRS).
• Bachelor’s degree in Recreational Therapy or related field or equivalent work experience.
• 1–3 years of experience providing recreational therapy to aging and frail populations.
• Ability to independently plan, implement, and evaluate therapeutic recreation interventions.

Responsibilities

• Conduct therapeutic recreation assessments to evaluate functional abilities and document findings per regulatory requirements.
• Develop and implement individualized and group therapeutic recreation interventions.
• Contribute to IDT care planning with assessment findings and SMART goals.
• Adapt activities to diverse functional levels, cultures, and preferences.
• Monitor progress and update care plans; document interventions and outcomes.
• Support safe programming within day center and community settings with proper supervision.
• Participate in care transitions by advising on home or facility-based therapeutic support.
• Maintain supplies and spaces to ensure a safe, welcoming environment.
• Engage participants with cognitive impairment or behavioral health needs using appropriate approaches.
• Identify opportunities to improve programming and workflows.
