Job summary

Habitat Health in California seeks an Activities Coordinator to support the well‑being of participants in its PACE program. You will plan and deliver therapeutic recreation, assess needs, and work within an interdisciplinary team to align with participant goals and regulatory requirements.

You will design monthly activity calendars, coordinate with therapists, and adapt programs for diverse abilities and backgrounds while maintaining safety and quality.

Qualifications

• 1–3 years experience providing recreational therapy or activity programming to aging and frail populations.
• Active CPR/BLS certification.
• Ability to plan, implement, and evaluate daily activities and socialization programs.

Responsibilities

• Conduct comprehensive recreational and social assessments and document findings per PACE requirements.
• Design and lead therapeutic group activities to support physical, cognitive, social, and emotional wellness.
• Create the Center’s monthly activity calendar and coordinate with therapists to meet goals.
• Collaborate with IDT to share findings and create SMART goals and interventions.
