Job summary

Habitat Health is seeking a motivated Recreational Therapist (CTRS) to support the PACE program, promoting engagement and functional ability for older adults. You will assess participants, plan activities, and collaborate with the interdisciplinary team to align with individual goals and regulatory requirements.

Responsibilities include conducting assessments, delivering personalized interventions, and contributing to care planning.

Qualifications

• Certified Therapeutic Recreation Specialist (CTRS) required.
• Bachelor’s degree in Recreational Therapy or related field or equivalent experience.
• 1–3 years of experience providing recreational therapy for aging/frail populations.
• Ability to plan, implement, and evaluate therapeutic recreation interventions.
• Experience conducting functional assessments and documenting progress.

Responsibilities

• Conduct therapeutic recreation assessments and document findings per PACE requirements.
• Develop and implement individualized and group therapeutic recreation interventions.
• Contribute to IDT care planning with assessment results and SMART goals.
• Adapt activities to diverse functional levels and cultural backgrounds.
• Monitor progress and update care plans; document outcomes.
• Ensure safe, engaging programs at the center and in the community.
