Job summary

Habitat Health in Compton, CA seeks an Activities Coordinator to support the wellbeing of participants in the PACE program. You will plan, lead, and assess recreational activities, coordinating with physical and occupational therapists to promote functional ability and social engagement.

You will conduct assessments, create care plans, and adapt programming for diverse backgrounds while maintaining safety and compliance.

Qualifications

• Experience providing recreational therapy to aging and frail populations.
• Active CPR/BLS certification.
• Ability to plan, implement, and evaluate daily activities.
• Experience documenting assessments and care plans.
• Strong communication and collaboration within the IDT.
• Adapt activities for diverse cognitive levels and backgrounds.
• Reliable and safe day-to-day scheduling and resource tracking.

Responsibilities

• Conduct comprehensive recreational and social assessments on time and document findings per PACE regulations.
• Design and lead therapeutic group activities to support physical, cognitive, social, and emotional wellness.
• Create the Center’s monthly calendar of activities engaging all participants.
• Identify community resources to support recreational and social needs and future programming options.
• Coordinate with Physical and Occupational Therapists to incorporate restorative goals.
• Contribute to IDT care planning with SMART goals and interventions.
• Adapt activities to diverse functional levels and cultural backgrounds.
• Monitor progress, update care plans, and document interventions and outcomes.
• Ensure safe and engaging programming with proper supervision and safety protocols.
• Maintain supplies, equipment, and activity spaces.
• Engage participants with cognitive impairment or behavioral health needs using appropriate approaches.
• Identify opportunities to improve programming and workflows.
• Perform related duties as assigned.
