Job summary

Habitat Health is seeking a Registered Recreational Therapist (CTRS) to support older adults in the PACE program. You will assess participants, design individualized and group interventions, and collaborate with the interdisciplinary team to align care plans with goals.

The role emphasizes safety, documentation, and engaging activities in center and home settings. The position offers an hourly wage of $34–$39, a target bonus of 5%, and comprehensive benefits.

Qualifications

• CTRS certification required.
• Bachelor’s degree or equivalent experience in Recreational Therapy.
• 1–3 years providing recreational therapy for aging/frail populations.
• Strong communication and collaboration within an IDT.
• Ability to plan, implement, and evaluate therapeutic interventions.

Responsibilities

• Conduct therapeutic recreation assessments and document findings per regulatory requirements.
• Develop and implement individualized and group interventions to improve functioning.
• Contribute to IDT care planning with assessment findings and SMART goals.
• Adapt activities for diverse functional levels and cultural backgrounds.
• Monitor progress and update care plans; document outcomes.
