Job summary

Adaptive Home Health is seeking Physical Therapist Assistants to deliver skilled PTA visits in patients' homes under PT supervision. You’ll focus on functional mobility, gait/balance, and home safety, documenting progress in Homecare Homebase (HCHB) and coordinating with the supervising PT.

Qualifications

• Active Texas PTA license in good standing.
• Strong clinical judgment and ability to work independently in the field (within PTA scope and under PT supervision).
• Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on treatment plans, visit cadence, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
• Set/track objective progress within PTA scope and communicate updates to the supervising PT.
• Drive multidisciplinary coordination: elevate changes in condition, align on orders, keep the care team moving.
• Coach patients and caregivers with practical home programs that get done.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
