Adaptive Home Health is seeking a Physical Therapist Assistant (PTA) to perform skilled visits in the home under a supervising PT across Texas. This full-time field-based role offers a productivity-based compensation model and a schedule of Monday–Friday with occasional weekends as needed.

You will deliver evidence-based interventions for mobility and safety, coordinate with the care team, and educate patients and caregivers.

Formación

• Active Texas PTA license required.
• Strong clinical judgment and independent field work in PTA scope.
• Clear documentation and communication with care team.
• Reliable follow-through on treatment plans and goals.
• Valid driver’s license and daily travel across territory.

Responsabilidades

• Deliver skilled PTA visits in the home under supervising PT’s plan of care.
• Execute treatment plans focused on mobility, gait, transfers, strengthening, and home safety.
• Set and track progress; communicate updates to supervising PT.
• Coordinate with care team to elevate changes in condition and maintain momentum.
• Coach patients and caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB) with defensible records.
