AccentCare is seeking a Territory Patient Admissions Coordinator in Oakdale, MN to manage referrals and coordinate with families, clients, and referral sources. This full-time, onsite role requires healthcare experience and EMR proficiency.

You will collaborate with agency staff and sales to ensure timely, accurate referrals and high-quality patient care, while navigating payor requirements and documentation in Epic or HCHB systems. This role supports compassionate, in-place aging care.

Qualifications

• High School or GED required.
• Minimum of one year of healthcare experience.
• Proficient in navigating and documenting within EMR systems; Epic and HCHB preferred.
• Experience dealing with Medicare/Medicaid and commercial payors preferred.

Responsibilities

• Collaborates with Referral Source, Sales teams, clients and local agency operations to collect information for a quality referral.
• Documents referral information, actions, and follow-up in the medical record.
• Ensures timely attention to referrals from eFax, phone, or portal/Hospital EMR.
• Reviews Eligibility Alerts and Responses for Medicare Clients where applicable.
• Requests H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team.
• Makes follow-up calls to clients/caregivers/referral sources to collect missing data and clarify expenses.
• Ensures accuracy and completeness of referral information for timely processing and quality patient care.

Location: Oakdale, Minnesota onsite in office

Position Type: Full-Time

Schedule: Monday - Friday 8am - 5pm

Compensation: $23.00 to $30.00 per hour

What You Need To Know

The Territory Patient Admission Coordinator establishes and maintains communication with the clients, family members and referral sources, in a given territory. This role is responsible for partnering with agency staff, referral sources, and sales to achieve optimal patient satisfaction & outcomes. The Territory Patient Admission Coordinator is responsible for the timely acceptance or decline of the referral based off the predetermined criteria set by the Agency and facilitates obtaining all information and documentation necessary to complete a quality referral. This position’s assigned territory, may encompass one or more states in AccentCare’s operation and may change with the needs of the business.

• Experience in dealing with a variety of payors in healthcare, e.g., Medicare, Medicaid, and commercial payers, preferred

Responsiblities

• Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
• Documents all referral related Information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable utilizes the agency’s software solution to facilitate the referral process by completing required workflow task within the Company’s established time frames.
• Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal.
• Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients.
• Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.
• Completes a "Follow Up" call to the Client/Caregiver/Referral Source to collect or confirm information. Example: confirms out of pocket expense, clarify teachable caregiver, and collect missing demographic and physician information.
• Demonstrates exceptional attention to detail by reviewing referral information for accuracy, completeness, and consistency, ensuring all required documentation is obtained and entered correctly to support timely processing and quality patient care.
