Infinity Hospice Care LLC is seeking a Patient Care Advocate to expand referral networks and promote hospice services across the Fort Worth area. You will lead targeted outreach to physicians, facilities, and care-at-home providers, and develop territory plans to grow referral sources and program utilization.

The role emphasizes relationship-building, timely communication, and collaboration with Intake and clinical teams to ensure high-quality end-of-life care and compliant service delivery

Formación

• Bachelor's degree in marketing or related field or equivalent professional experience.
• Two years hospice sales or related industry experience with growth.
• Proficiency in building professional relationships with internal and external teams.
• Proficient in Microsoft programs for communications, presentations, tracking, and reporting.
• CRM experience preferred.
• Excellent oral communication skills.
• Self-directed with minimal supervision.

Responsabilidades

• Develop territory plans to achieve growth goals.
• Grow referral sources including physicians, facilities, and community agencies.
• Make effective sales calls and presentations to targeted referral sources.
• Communicate program offerings and coordinate with Intake and clinical teams.
• Maintain up-to-date referral source profiles and activity logs.
• Provide guidance to patients and families about hospice services as needed.
