Job summary

Key People is seeking a Clinical Manager to oversee patient care services and clinical staff in our California operations. The role ensures safe, effective care delivery, CHAP/regulatory readiness, and robust QAPI initiatives.

The ideal candidate holds an RN license, at least 5 years of RN experience with 2+ years in a supervisory capacity in Home Health, and must be CPR certified with a driver’s license and ability to travel to all locations.

Qualifications

• Must be a registered nurse licensed to practice in the state of operation.
• Minimum 5 years RN experience with 2+ years supervisory experience in Home Health.
• Experience managing RN services in the home, community, or clinic is preferred.
• Must possess and maintain CPR certification while employed in a clinical role.
• Must be a licensed driver with insured vehicle and be able to travel to all business locations.

Responsibilities

• Oversee Operations: Manage day-to-day clinical and administrative office operations, serving as a primary point of contact for staff and patients during operating hours.
• Ensure Regulatory Compliance: Direct care delivery to align with professional standards, agency policies, and state and federal regulations.
• Provide Leadership & Backup Support: Serve as acting leader in the absence of the Territory Administrator and provide coverage for PCM and CSS roles as needed.
• Coordinate Patient Care: Manage intake and referral process with interdisciplinary team to develop and execute individualized care plans.
• Drive Quality & Assessment: Evaluate care quality, assess ongoing patient needs, and collaborate with clinicians and physicians on transfers and discharges.
• Manage OASIS & Discharge Data: Review discharge assessment data, edit/lock Discharge OASIS forms, and oversee discipline-only discharges.
• Lead QAPI: Direct QAPI initiatives, track incidents, analyze trends, and implement corrective actions.
• Maintain Survey Readiness & Analytics: Maintain accreditation/survey readiness, conduct audit reviews, and manage HHCAHPS survey distribution.
• Oversee Emergency Preparedness: Lead EPP planning, activation, reporting, and ongoing event monitoring.
