Job summary

Brilliant Corners in Gardena, CA seeks a Case Manager for Permanent Supportive Housing Project Based (PSH-PB). You will provide intensive, client-centered case management for up to 20 homeless individuals at the project site, focusing on health, mental health, and housing stability.

This role emphasizes a "whatever it takes" approach with strong collaboration among partners, housing staff, and service providers to foster stability.

Qualifications

• At least 1 year of experience with homeless individuals and a Bachelor’s Degree in Social Work, Psychology, Sociology or related field OR at least 2 years of direct mental health/intensive case management experience.
• Experience with individuals who have mental illness, chronic health issues, and/or substance use disorder.
• Experience working at a project-based site preferred.

Responsibilities

• Accept, screen, and process referrals from HSH and partners.
• Conduct outreach and engagement in the community to locate referrals.
• Assess and reassess with psychosocial evaluation to determine eligibility and needs.
• Develop and implement individualized case management plans for housing stability.
• Provide housing process support including unit identification and lease-up coordination.
• Educate clients on tenant rights, eviction prevention, and advocacy.
• Linkage to health, benefits, and services.
• Attend regular meetings and case conferences; coordinate with onsite property management.
