Job summary

Boston Children’s Hospital is seeking a care coordination assistant to support patients with SLO syndrome and Lysosomal Storage Disorders. You will help with scheduling reviews and resource navigation.

Responsibilities include follow-up calls at 1 week and 1 month, coordinating with MDs/NPs, identifying overdue follow-ups, collecting data on task completion, and updating the resource guide to improve patient care.

Responsibilities

• Review upcoming schedules for patients with SLO syndrome and Lysosomal Storage Disorders (cross checking against clinic lists)
• Conduct follow-up phone calls with families 1 week and 1 month after the visit to review what the provider listed as action items, sending follow-up portal messages
• Follow up with the MDs/NPs if new questions arose or if families had medical questions
• Review patient lists for patients that are overdue for follow-up and messaging the admin team to get those patients scheduled
• Collect data on number of tasks/time to completion/use of access care coordinators/etc.
• Other care coordination tasks such as looking for resources and updating resource guide
