Boston Children's Hospital is seeking a care coordination professional to support scheduling, family follow-ups, and multidisciplinary communication. You will liaise with physicians, nurses, and the admin team to ensure timely patient follow-ups and resource access.

Responsibilities include reviewing schedules for complex cases, contacting families after visits, and maintaining up-to-date resource guides, with emphasis on effective coordination and patient support.

Responsibilities

• Review upcoming schedules for patients with SLO syndrome and related Lysosomal Storage Disorders.
• Conduct follow-up calls with families 1 week and 1 month after visits to review action items and send follow-up portal messages.
• Follow up with MDs/NPs if questions arise from families.
• Review patient lists to identify overdue follow-ups and coordinate scheduling with admin team.
• Collect data on number of tasks, time to completion, and use of access care coordinators.
• Look for resources and update the resource guide.
