Job summary

Covenant Living Communities and Services is seeking a Scheduling Coordinator to manage patient visit schedules for home health and hospice services. You will coordinate with nursing staff, patients, and families to ensure timely visits and accurate records.

The ideal candidate has strong organizational and communication skills, HIPAA awareness, and 3+ years in home health scheduling. Proficiency in Microsoft Office and Netsmart is preferred, with a high school diploma or higher as required.

Qualifications

• High School Diploma or higher; AA/AS preferred.
• 3 years Home Health and/or Hospice scheduling experience preferred.
• Strong organizational and communication skills; able to multitask under pressure.
• Knowledge of medical terminology and patient care protocols.

Responsibilities

• Coordinate and schedule patient visits with nursing staff based on patient needs and care plans.
• Schedule referrals for Home Health and Hospice in a timely manner.
• Maintain accurate patient records and documentation.
• Communicate with patients, families, and healthcare professionals to manage appointments.
