Job summary

MJHS is seeking a Telephonic Care Manager to support Medicare and Medicaid members via phone. You will assess medical, psychosocial, physical and spiritual needs and develop, implement, and monitor person-centered service plans in collaboration with the interdisciplinary team, PCP, members or their representatives, and specialists.

Qualifications

• Bachelor’s degree in social work or related field required.
• MSW or LMSW preferred.
• 3–5 years clinical social experience in long-term care, managed care, home care, geriatrics or hospice preferred.
• Prior experience in an interdisciplinary service delivery environment preferred.
• Working knowledge of Medicare and Medicaid regulations preferred.
• Current LMSW in New York State preferred.

Responsibilities

• Provide telephonic support to members’ medical, psychosocial, physical and spiritual needs.
• Develop, implement, and monitor person-centered service plans.
• Ensure continuity of care across settings by collaborating with the interdisciplinary team, PCP, member representatives, and specialists.
• Educate members on disease processes, preventive health, and quality of life interventions.
• Adhere to CMS/DOH regulations and policies.
