Job summary

MJHS in the United States seeks a Social Work Care Manager to support telephonic member engagement across medical, psychosocial, physical and spiritual needs, developing person-centered care plans.

You will collaborate with interdisciplinary teams, PCPs, and member representatives; educate members on disease processes, preventive health and quality of life interventions; ensure CMS/DOH compliance.

Qualifications

• Bachelor’s degree in social work or related field required; MSW or LMSW preferred.
• 3–5 years clinical social experience in long-term care, managed care, home care, geriatrics or hospice.
• Prior experience in an interdisciplinary service delivery environment.
• Current LMSW in New York State preferred.

Responsibilities

• Provide telephonic support for members’ medical, psychosocial, physical and spiritual needs.
• Develop, implement, and monitor person-centered service plans across care settings.
• Coordinate continuity of care with interdisciplinary team, PCP, member representatives and specialists.
• Educate members on disease processes, preventive health and quality of life interventions.
• Adhere to CMS/DOH regulations and policies.
