Job summary

MJHS’s Elderplan and HomeFirst Medicare/Medicaid plans seek a Telephonic Social Work Care Manager to address members’ medical, psychosocial, physical and spiritual needs.

You will develop, implement and monitor person-centered service plans, ensuring care continuity across settings by collaborating with the interdisciplinary team, PCP, and specialists. Education on disease processes and preventive health is essential, with CMS/DOH adherence.

Qualifications

• Bachelor’s degree in social work or related field required, MSW or LMSW preferred.
• Minimum three to five years clinical social experience in long term care, managed care, home care, geriatrics or hospice preferred.
• Prior experience in an interdisciplinary service delivery environment preferred.
• Working knowledge of Medicare and Medicaid regulations preferred.
• Current LMSW in New York State preferred.

Responsibilities

• Provides telephonic support for members’ medical, psychosocial, physical and spiritual needs.
• Develops, implements, and monitors person-centered service plans.
• Ensures continuity of care across settings by collaborating with the interdisciplinary team, PCP, member representatives and specialists.
• Educates members on disease processes, preventative health, and quality of life interventions.
• Adheres to CMS/DOH regulations and policies.
