Job summary

MJHS is seeking a telephonic Social Work Care Manager to support Elderplan and HomeFirst members, addressing medical, psychosocial, physical and spiritual needs.

You will develop and monitor person-centered service plans, coordinate with the interdisciplinary team, PCP, and members to ensure seamless continuity of care across settings. CMS/DOH regulations guide all activities, and LMSW or MSW credentials are preferred.

Qualifications

• Bachelor’s degree in social work or related field required; MSW or LMSW preferred.
• 3–5 years clinical social work experience in long term care, managed care, home care, geriatrics or hospice.
• Experience in an interdisciplinary service delivery environment preferred.
• Working knowledge of Medicare and Medicaid regulations preferred.
• Current LMSW in New York State preferred.

Responsibilities

• Provide telephonic support for members’ medical, psychosocial, physical and spiritual needs.
• Develop, implement, and monitor person-centered service plans.
• Ensure continuity of care across settings by collaborating with the interdisciplinary team, PCP, member/representatives, and specialists.
• Educate members on disease processes, preventative health, and quality of life interventions.
• Adhere to CMS/DOH regulations and policies.
