Job summary

MJHS is seeking a SW Care Manager to coordinate telephonic support and care planning for Medicare/Medicaid members in New Jersey. You will assess medical, psychosocial, physical, and spiritual needs and develop person-centered service plans.

You will collaborate with the interdisciplinary team, PCPs, and member representatives to ensure seamless care across settings, educate members about disease processes and preventive care, and ensure adherence to CMS and DOH regulations.

Qualifications

• Bachelor’s degree in social work or a related field is required.
• MSW or LMSW is preferred.
• 3–5 years of clinical social work experience in long-term care, managed care, home care, geriatrics, or hospice.
• Experience in an interdisciplinary service delivery environment is preferred.
• Familiarity with Medicare and Medicaid regulations preferred.
• Current LMSW in New York State preferred.

Responsibilities

• Provide telephonic support addressing members' medical, psychosocial, physical, and spiritual needs.
• Develop, implement, and monitor person-centered service plans.
• Ensure continuity of care across settings by collaborating with the interdisciplinary team, PCP, member representatives, and specialists.
• Utilize benefit structures and community resources to meet member needs.
• Educate members on disease processes, preventive health, and quality of life interventions.
• Adhere to CMS/DOH regulations and policies.
