Job summary

MJHS in New Jersey seeks a Social Work Care Manager to support telephonically the medical, psychosocial, physical and spiritual needs of members. You will develop and monitor person-centered service plans and coordinate care across settings with the interdisciplinary team.

Role emphasizes CMS/DOH compliance, Medicare/Medicaid knowledge, and collaboration with PCPs, member representatives and specialists to enhance quality of life.

Qualifications

• Bachelor’s degree in social work or related field required.
• MSW or LMSW preferred.
• 3–5 years clinical social work experience in long term care, managed care, home care, geriatrics or hospice preferred.
• Working knowledge of Medicare and Medicaid regulations preferred.
• Experience in an interdisciplinary service delivery environment preferred.
• Current LMSW in New York State preferred.

Responsibilities

• Provide telephonic care addressing members’ medical, psychosocial, physical and spiritual needs.
• Develop, implement and monitor person-centered service plans.
• Ensure continuity of care across settings by collaborating with the interdisciplinary team, PCP, member representatives, and specialists.
• Educate members on disease processes, preventative health and quality of life interventions.
• Adhere to CMS/DOH regulations and policies.
